The undersigned(s) being the lawful parent(s) and/or guardian(s) of the above child, hereby consent to the participation by the child in all day care activities conducted by Organic Connections childcare program and to the participation of the child in all events related to said activities.
The undersigned(s) hereby further authorize(s) emergency transportation by either day care personnel or if necessary by ambulance or other emergency vehicle.
If there is no medical emergency, the childcare staff will first use reasonable efforts to contact the parent(s) and /or guardian(s) before administering or authorizing any treatment.
The Organic Connections childcare program is consistently well supervised. However, accidents do happen. The undersigned(s) assume(s) all risk of injury or harm to the child associated with participation in the childcare and agree(s) to release, indemnify, defend and forever discharge Organic Connections childcare program and its staff, board, and volunteers of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child's participation in the childcare.
Carol Brucks is a mother of four from Wilcox, Sk. Born and raised in Regina, she attended the U of R and received her Bachelor of Elementary Education in 2007. After many years working as an Assessment coordinator/tutor at Sylvan Learning Center or Private Tutor; Carol became solely focused on home-education for her children all while volunteering as Children’s Church Teacher at her local Church, coordinating Summer Kids Day Camps for the past 5 years in Milestone, Sk, and the Board Chair for the local library. She currently assists with the kids’ summer programming at the library (alongside many others) and volunteered her time at Echo Lake Bible Camp as a “Camp Mom”. She enjoys working with children of all ages and puts their safety above all else.
Carol’s job is to make sure that the children have a great time at this conference, and she will be doing just that! It's not hard for her because each day of fun activities are planned so they can enjoy themselves.
